Guide to Changing Subject Combinations and Colleges in SELFORM System

Introduction

SELFORM system enables Form Four graduates to update their information and change subject combinations, schools, or middle-level colleges according to examination results.

Eligible Students

• Form Four Graduates
• Students changing Form Five combinations
• Applicants for Middle-Level Colleges

How to Log in to SELFORM

1. Connect your device to the internet.
2. Open a web browser such as Chrome.
3. Visit: https://selform.tamisemi.go.tz

First Time Registration

• Click “For Candidates, Click here to Register”.
• Enter Index Number (Example: S0101.0020.2018).
• Fill surname and year of birth.
• Answer the security question.

Creating Password

Create a password you will use every time you log into the system. After confirmation, use your username and new password to access the dashboard.

Changing Combination or College

1. Login into your dashboard.
2. Open the Choices Selection section.
3. Select new school or subject combination.
4. Save changes.
